We utilize local staff from our community and participate actively in community events, and sponsor events in the local community as permitted. We have over 20 years combined services to our patients. We provide limited indigent services to the community as permitted by State and Federal Regulations.

Mountain Creek Home Health and Country Plains Home Health is a 501 C-3 Non-For-Profit organization that serves rural Southern Colorado. Employees may qualify for the Rural Student Loan Forgiveness and Deferment programs through the Federal Student Loan Programs. Some employees can pay their student loans in full by making only 10 years of Income Based Repayment Program "IBR".
